For appropriate instructions, please contact and furnish the following information to Product Evaluation and Regulation (PER) Unit:
Common/generic name(s) of active ingredient(s)
Amount/concentration/strength/ of each active ingredient
Product label and package insert
Recommended use(s)/indication(s) of the product.
